On 26 February 1972, a coal-waste impoundment failed above Buffalo Creek in Logan County after heavy rain. Water and slurry cascaded through two lower impoundments and became a black flood moving down a narrow inhabited valley. It killed 125 people, injured more than 1,100, and left about 4,000 homeless while destroying or damaging hundreds of homes.

Pittston Coal's Buffalo Mining Company had built successive refuse dams from loosely dumped mine waste across Middle Fork. State and federal inspectors raised concerns about drainage and stability, but regulatory jurisdiction was fragmented and enforcement weak. Communities, roads, schools, and homes occupied the constricted creek bottom immediately below.

Several days of rain raised the upper pool behind Dam No. 3. Water overtopped or penetrated the refuse structure on Saturday morning, and the dam collapsed. Its release overwhelmed Dams Nos. 2 and 1, combining stored water, silt, coal fines, and debris into a wave that reached communities with little effective warning.
