The Deepwater Horizon Disaster
On 20 April 2010, hydrocarbons surged from BP's Macondo well onto the Deepwater Horizon drilling rig, ignited, and caused explosions and fire. Eleven workers died and seventeen were seriously injured. The rig sank two days later, leaving the well flowing at the seafloor for eighty-seven days and releasing about 4.9 million barrels of oil in the largest marine oil spill in United States history.
Background
The Macondo project was behind schedule and over budget in a deepwater environment where well control depended on cement, pressure interpretation, casing, barriers, crew coordination, and a subsea blowout preventer. BP, Transocean, Halliburton, contractors, and federal regulators operated through divided responsibilities. Cost and time pressures interacted with a safety regime focused more on worker injury than catastrophic process risk.
What happened
During temporary abandonment, a negative-pressure test produced anomalous results that were misread as a bladder effect. Workers replaced heavy drilling mud with seawater before confirming secure cement barriers. Oil and gas flowed up the well, overwhelmed separation systems, reached ignition sources, and disabled control. Blowout-preventer components failed to seal the buckled drill pipe.

Aftermath
Most people evacuated by lifeboat or jump, while the eleven missing workers were never recovered. Remotely operated vehicles failed to activate the blowout preventer. Oil spread at the surface and depth as responders used booms, skimming, burning, shoreline cleanup, and unprecedented dispersant volumes. Fisheries closed, wildlife was oiled, and coastal livelihoods and mental health suffered.
Long-term consequences
A capping stack stopped the flow on 15 July and a relief well sealed Macondo in September. BP pleaded guilty to criminal charges and, with civil settlements, paid tens of billions for penalties, cleanup, restoration, and claims. Federal offshore regulation was reorganized and drilling rules strengthened, though industry expansion and regulatory capacity remain contested.

It demonstrates why high-hazard work needs independent verification, stop-work authority, process-safety indicators, and equipment designed for the ways structures deform during the exact emergency they must control.
What remains today
The wreck rests on the seafloor near the plugged well. Memorials in Louisiana and Mississippi, the Ocean Star museum, recovered equipment, inquiry records, ecological monitoring plots, restoration projects, photographs, survivor testimony, and ongoing Gulf research preserve human and environmental evidence.

The federal estimate of 4.9 million barrels includes about 800,000 barrels captured and carries measurement uncertainty. Ecological injury varies by species, place, depth, and time; absence of visible surface oil does not define full recovery. Responsibility was shared but not equal, and legal allocations differ from causal narratives.
